Natural Doesn't Mean Safe: What to Know Before You Try Peptides

Ask the Experts, Part 2 of 3 In Part 1 of this series, we asked three doctors, Dr Haytham Salem, Dr Tatjana Pavicic, and Dr Jennifer Kaur, to explain what peptides actually are, and why they're not the same as steroids or growth hormone. Now we're moving into the questions that matter most once you're actually considering trying one: Is "natural" the same as "safe"? Does a peptide cream do the same job as an injection? And why does everyone seem to expect results by next Tuesday?

Myth: "Because they're natural, peptides are automatically safe."

This is where all three doctors were most direct, and most united.

"I always smile when I hear someone say, 'It's natural, so it must be safe.' Nature gives us wonderful medicines, but it also gives us poisonous mushrooms." Dr Haytham Salem

Dr Pavicic takes it a step further with an example that's hard to forget:

"Botulinum toxin, the active ingredient in anti-wrinkle injections, is entirely natural. So is arsenic. What actually decides safety is the dose, the purity, how sterile it is, how it enters the body, and whether it has been properly studied and prescribed."

Her conclusion is blunt: a peptide made in an unregulated lab with no quality checks isn't safe just because its structure resembles something already in your body. Dr Salem echoes this, noting that safety comes down to "the right patient, the right indication, the right dose, and a high-quality product," which is exactly why he believes peptide therapy should always be doctor-led.

Are peptide injections actually approved?

This is a question we hadn't originally planned to spotlight, but Dr Pavicic's answer was too useful to leave out. She draws a sharp line between properly regulated peptide medicines and what's often sold online:

"GLP-1, short for glucagon-like peptide-1, is a natural hormone your gut releases after you eat, and it is itself a peptide. The well-known weight-loss and diabetes medicines, such as semaglutide and tirzepatide, are peptide drugs that copy that hormone... They went through years of clinical trials and are approved for specific conditions. That is what a properly approved peptide looks like."

Compare that, she says, to the peptides "promoted online for anti-ageing, fat loss, muscle gain or recovery," many of which are unapproved for those uses, sometimes labelled "research use only" despite being marketed directly to consumers. Her rule of thumb for any patient:

"An injectable peptide should be prescribed by a licensed doctor, dispensed by a licensed pharmacy, and approved for use in the country where you are being treated."

Myth: "Topical peptide skincare works the same way as injectable peptides."

If you've ever wondered whether that peptide serum in your skincare drawer is doing the same thing as an injectable, the answer from all three doctors is a clear no.

Dr Salem frames it simply:

"A peptide cream can certainly support skin quality and collagen signalling in the outer layers of the skin... Injectable prescription peptides, on the other hand, are designed to influence specific biological pathways throughout the body or in targeted tissues. They're not competing treatments, they're simply designed for different purposes."

Dr Pavicic goes deeper on why topicals are limited; it comes down to the skin barrier itself:

"Most peptides are fairly large, water-loving molecules, and the top layer of your skin is built specifically to keep those out. That is why you often see the word 'palmitoyl' at the start of a peptide name on an ingredient list. A tiny fat tail has been added to help the molecule slip through the barrier."

Her honest verdict: a well-formulated peptide serum is "a helpful supporting player" that can improve skin quality over time, but it doesn't replace sunscreen or retinoids, and it certainly won't replicate the effect of an injection.

Myth: "If a little works, more works better."

Every doctor we spoke to shut this one down immediately.

"Medicine rarely works like that... With peptides, the goal isn't to take as much as possible; it's to find the dose that gives your body the right signal. Too much may increase side effects or simply stop giving additional benefit." Dr Salem

Dr Kaur points out that trendy dosing strategies you see online aren't universal:

"There's a lot of discussion online about 'microdosing' or 'maximum dosing,' but these aren't universal strategies that apply to every peptide or every person... Taking more doesn't necessarily make a peptide work better, it may simply increase the risk of side effects or move you outside the therapeutic window."

Dr Pavicic's plant analogy is one we love:

"It is like watering a plant. The right amount helps it grow, and drowning it does not make it grow faster. With injectable peptides the stakes are higher, because taking more than prescribed, with no doctor watching, can create real risks."

Myth: "You'll see results immediately."

Patience, it turns out, is non-negotiable.

"We're living in a world where people expect Amazon Prime results from human biology. Unfortunately, or perhaps fortunately, our bodies don't work like that." Dr Salem

Dr Pavicic gets specific about topical timelines:

"With a topical peptide, expect to notice a difference in skin quality after about 8 to 12 weeks of consistent use, and sometimes longer... Many people give up at week three because they were expecting the instant, filtered look of a photo rather than a slow biological change."

Dr Kaur adds a broader point worth sitting with: peptides can't do the job alone.

"Peptides aren't a substitute for sleep, nutrition, exercise, or addressing the underlying cause of symptoms. If those foundations aren't in place, even the right peptide may not deliver the results someone is expecting."

What to take from all this?

Natural doesn't mean risk-free, a serum can't do an injection's job, more is rarely better, and real change takes weeks or months, not days.
