From Stroke Recovery to Elite Performance: Dr Craig R. Cook on Redefining Brain Health

When The Brain & Performance Centre opened in Dubai, the ambition was straightforward: build a genuine destination for brain health, backed by world-class medical talent and technology, under one roof. Years on, it has become the first facility in the Middle East, and one of only three outside the US, to receive UHMS accreditation for hyperbaric medicine, treating everything from paediatric cerebral palsy to executive burnout to elite athletic performance. Its client list includes names like Tommy Fleetwood and Hamzah Sheeraz, alongside patients managing serious neurological conditions, all under the same clinical standard. We sat down with Dr Craig R. Cook, CEO of The Brain & Performance Centre and a DP World company, to talk about what it took to earn that accreditation, how the Centre balances medical credibility with its growing performance brand, and where hyperbaric oxygen therapy is headed next.

1. What was the original vision behind the Centre, and how has that vision evolved since you opened?

The original idea was to build a genuine destination for brain health, somewhere that could bring cutting-edge technology and world-class medical talent under one roof and treat both children and adults with the same level of rigour. Dubai was the perfect place to do that. No legacy systems to work around, and real ambition to build something new in healthcare.

What's evolved is our sense of who this is actually for. We assumed most of our clients would be recovering from something; a stroke, a brain injury or cerebral palsy and a huge part of what we do is still that. More than 2,000 clients have now been treated using this approach, with over 70,000 hyperbaric sessions delivered. That experience has taught us that brain health is not only about recovery; it is also about performance and maintaining cognitive health over time.

2. You're the first facility in the Middle East, and one of only three outside the US, to receive UHMS accreditation for hyperbaric medicine. What did it take to reach that standard, and why does that accreditation matter for someone considering treatment here?

It took years of building our clinical infrastructure to a standard we knew would eventually be audited against the best hyperbaric centres in the world. The Undersea and Hyperbaric Medical Society doesn't just check your equipment. They evaluate your safety protocols, physician oversight, staff certification, documentation and outcome tracking, all of it.

For a patient, that accreditation answers the one question that actually matters. Who is responsible for my treatment, and by what standard are they being held? HBOT is a real medical intervention, it changes physiology, and without proper oversight it carries real risk. UHMS accreditation is what separates a facility that treats HBOT as serious medicine from one that treats it as a wellness add-on. For a parent bringing in a child, or someone managing a serious neurological condition, that distinction matters enormously.

3. The Centre treats an unusually wide range of conditions, from paediatric cerebral palsy to executive burnout to elite athletic performance. How do you build a single programme model that genuinely serves such different clients, and where does it need to flex most?

The underlying principles are consistent, but each client's needs are different. For our comprehensive programmes, the model typically includes a structured assessment, an intensive HBOT programme supported by cognitive and physical training, and a post-programme reassessment. That gives us a consistent framework while allowing the programme to be personalised around the individual.

The greatest flexibility is around that framework. A paediatric case requires a very different intake process, pace and level of parental involvement from an athlete focused on performance data or an executive who values discretion and efficiency. That is why we have distinct clinical, paediatric and Peak Performance pathways: they share the same standards and medical oversight but are shaped around very different people.

4. As a DP World company, how does that backing shape the business, whether it's investment in technology, the ability to attract international medical talent, or the standards you're held to?

Being part of DP World shapes nearly every dimension of our business, and it does so in ways that have been genuinely transformative. DP World operates critical infrastructure at scale across more than 80 countries, and that same long-term, world-class operational mindset carries directly through to us. It has given us the freedom to invest ambitiously in advanced hyperbaric technology, diagnostic capabilities and the facility itself, without the short-term pressures that so often limit promising healthcare ventures.

That backing has also been a powerful advantage in attracting exceptional talent from around the world. When we tell a physician or clinical team member that they would be joining a facility backed by DP World, it carries real credibility and opens doors that a standalone clinic simply could not open on its own. Just as importantly, it raises our own standards. We are held to the same corporate governance, safety and reporting standards as any DP World company, and that discipline is a large part of why we were able to pursue and achieve an accreditation as demanding as UHMS. Being part of the DP World family does not just support what we do. It elevates it, and it is a large part of why we are able to deliver world-class care to the standard we do today.

5. You describe results as "clinically verifiable" and track patients before and after treatment. What does that outcome-tracking process actually look like, and how do you ensure the data holds up to outside scrutiny?

For clients enrolled in our comprehensive programmes, the process begins with a three-day baseline assessment led by physicians, neuropsychologists, physiotherapists and dietitians. That gives us measurable data before the programme begins. At the end, we repeat the same assessments, so we are comparing like with like, and we continue tracking progress through remote monitoring and in-person follow-ups.

To make sure that data actually holds up, we don't just mark our own homework. We use standardised, validated clinical tools rather than anything proprietary, and we push toward independent research where we can.

6. You've attracted high-profile clients like Tommy Fleetwood and DP World's own executives through Peak Performance, alongside patients dealing with serious neurological conditions. How do you balance being a wellness and performance brand with being a credible medical facility?

We do not see performance and medical credibility as competing identities. We are a medical facility that also supports performance and long-term cognitive health. Peak Performance is built on the same clinical standards as our medical programmes; the difference is the goal. An athlete or executive may be focused on performing well and maintaining cognitive health over time rather than recovering from illness, but the physicians, technology, safety oversight and measurement remain the same.

Recognisable names such as Tommy Fleetwood, Hamzah Sheeraz and Dylan Hartley have helped make the work more visible, but the standard does not change with the client. That consistency allows us to serve high performers without compromising our credibility as a medical facility.

7. Hyperbaric oxygen therapy is central to almost everything you do. What made HBOT the foundation of the Centre's approach, and how do you respond to scepticism from people who see it as an emerging or unproven field?

HBOT became the foundation of everything we do because the underlying science is genuinely well established. Increasing the amount of dissolved oxygen carried in the blood under pressure has decades of robust research behind it, from wound healing to decompression illness to radiation injury recovery. Building on that strong foundation, we've focused much of our clinical work on the expanding evidence for its role in neurological repair, including traumatic brain injury and stroke recovery. This is a genuinely exciting and fast-advancing area of medicine, and we're proud to be among the facilities helping to lead that research forward.

My answer to anyone who questions HBOT is simple: look at the programme, the medical oversight and the outcome data behind it. We are proud to be the first UHMS-accredited hyperbaric facility in the Middle East and one of only three outside the United States. That accreditation verifies the standards under which we deliver care. Separately, our approach is informed by published, peer-reviewed evidence, including a randomised controlled study in Scientific Reports on HBOT for paediatric post-concussion syndrome. Healthy scepticism is valuable because it pushes the field towards stronger evidence, clearer standards and greater accountability.

8. Looking ahead, where do you see the Centre expanding, whether that's new indications you treat, new partnerships like the one with the UAE Athletic Commission, or new markets beyond Dubai?

The future of the Centre is genuinely exciting, and it's one built on growth in every direction at once. We're continuing to expand the range of neurological and paediatric conditions we treat as the evidence base evolves, while deepening the research partnerships that allow us to publish rigorous, peer-reviewed results rather than simply adding new services for their own sake. That commitment to science will always be the engine behind everything else we do.

At the same time, partnerships such as our work with the UAE Athletic Commission are an important part of our next chapter. As one of the Commission's approved service providers, the Centre provides the mandatory pre-fight neurological MRI assessment, while the broader comprehensive assessment remains optional. This strengthens the information available before competition and can support more informed follow-up where an injury occurs. The wider goal is to strengthen athlete safety and raise standards of care across combat sports in the UAE.

Beyond that, our ambition is to continue evolving the way brain health is delivered. As the science advances, so too will the range of ways we can support people, whether they're recovering from neurological injury, striving for peak performance or looking to maintain cognitive health as they age. We'll continue investing in research, strategic partnerships and clinical innovation, while exploring opportunities to extend our model beyond Dubai where it can deliver meaningful impact. Whatever comes next, our commitment to scientific rigour, measurable outcomes and exceptional clinical care will remain unchanged.
